Complete Part I (Tax)
If you owe the Alternative Minimum Tax, complete Form 6251 and enter the result on line 1. If you received excess advance Premium Tax Credit payments, complete Form 8962 and enter the repayment amount on line 2. Add these for the Part I total on line 3.
Complete Part II (Other Taxes)
Enter self-employment tax from Schedule SE on line 4. Report unreported Social Security/Medicare tax from Form 4137 or Form 8919 on lines 5-6. Enter additional tax on IRAs or retirement plans from Form 5329 on line 8. Include household employment taxes from Schedule H on line 10.
Calculate totals and transfer to Form 1040
Add all amounts in Part II for the total on line 21. Transfer the Part I total (line 3) to Form 1040, line 17, and the Part II total (line 21) to Form 1040, line 23.
